Our convalescence and post-hospital care service provides short-term, intensive support to help you regain confidence and independence at home, rather than staying in hospital longer than necessary or moving into a care home.

Care can be arranged quickly around a discharge date and stepped up or down as your recovery progresses, so you always have the right level of support at the right time.
